Step 1 - Contact us - All we need to setup an initial meeting with you and your loved one is your name, phone number and answers to a few brief questions. Once we have this information we will schedule a meeting with you and one of our Care Managers in your home.
Step 2 - The Initial Meeting - We will answer your questions about our Home Care Services and briefly assess your family member’s needs. The Care Manager assesses their individual needs and records physical, lifestyle and personal preferences. We will then conduct a free senior home safety check and provide you with recommendations on how to keep the home safe for your family member. Additionally, the Care Manager will present the family with a detailed service plan and service packages that would be appropriate with pricing.
Step 3 - Agreement Signing - Once you agree to move forward with our care services we will need you to review, sign and date our Service Agreement and return it to us. Once this is completed we will decide on a date and time for our services to begin. We also will require an initial payment at the start of care for the first 2 weeks of services (we will provide you with exact amount).
Step 4 - Start of Service - The Care Manager will arrive to the Client’s home with the Caregiver to make introductions and to walk the Caregiver around the home and provide the Caregiver with a detailed service plan of activities and tasks to be completed on each service visit. The Care Manager will leave after the Client and Caregiver are comfortable that all the details needed to provide care assistance to the Client have been discussed.
Step 5 - After the First Day of Service - The Care Manager will contact the family and the Client to make sure they are pleased with the assistance that was provided on the first day of care. The Care Manager will then review the dates and times of future service visits to make sure they are accurate with the Family’s understanding.
A billing statement will be sent out twice a month, once for the 1st day of the month through the 15th day of the month and again for the 16th day of the month through the last day of the month. Payments are due upon receipt of each billing statement.
Step 6 - Ongoing Monitoring - Your Care Manager will contact you and your family routinely to discuss your Caregiver services. Additionally, they will perform periodic visits to the home to assess your Caregiver, the current level of care and will then determine if changes are needed. Your satisfaction with our services is our commitment and priority.
